Colleen Sullivan-Moore RN, MS is a native New Mexican with 30 years experience as a nurse. She has specialized in breast care for 14 years. She serves as a personal navigator, acting as liaison to enhance the quality and continuity of care for breast cancer patients.
Nurse Navigator
Understanding what resources and services are available, being able to access and in order to utilize them can be a challenge for anyone facing breast cancer. The Nurse Navigator will perform an initial assessment to compile education material for effective learning to support the breast cancer patient in problem solving, with individualized breast cancer information. We recognize every woman may have different educational needs based on age, reproductive history and gender. Each newly diagnosed patient will receive a “Breast Cancer Toolkit” to assure reliable information for effective learning in an informal supportive atmosphere. We also have a “Big Sisters First Aid Kit” which is geared toward premenopausal women. Our goal is to assist patient understanding about the impact of the diagnosis, to have knowledge of the community support services and non profits available to support their recovery.
Case Management
We provide individual case management services for our patients. Directing patients to appropriate resources and professionals. We connect patients to cancer counselors, peer support, support service for children, and legal services. We support, partnering, sharing and involvement of the community healthcare providers in breast health and successful outcome for our patients.
The Nurse Navigator will assist breast cancer patients with accessing appointments, support services and resources needed regarding insurance and financial issues. She will provide patient education about technique to improve patient doctor communicaiton.
Education Program
Our education program was designed to be multifaceted, didactic, and culturally sensitive and is available at no cost to our patients. We acknowledge the need for our services to be interactive and self directed. We understand the diagnosis of breast cancer affects the entire family. Helping patients and their families understand the impact of the diagnosis, treatment options, will enhance their experience. We pride ourselves on giving special consideration to the entire family. We believe these services improve clinical outcomes and learning is improved by the involvement of our patients.
Education material is in English, Spanish and literacy as appropriate for our patients. We are available to answer questions by phone or in person.
We pride ourselves at maintaining communication with breast cancer patients to check progress through the course of their experience to help assure their needs are being met. Ongoing classes are available on behalf the community and patients are ongoing and updated.
